Get Ready for Your Appointment!

To help you get ready for your appointment with your Housing Advisor, please send us the forms and information listed below.  If you don't have access to a computer, no worries, plan to stop by the office in advance of your appointment to complete the forms and drop off your information.  These are documents you'll also need to have available for your meeting with your mortgage loan officer.

At your appointment, you'll review your credit report, make sure you are comfortable with a proposed mortgage payment, explore down payment assistance programs, and if you're interested, enroll in the matched savings program.  All your questions will be answered.  Appointment fee to cover cost of credit report and materials:  $25.00

  3. Child Support Statement
  4. 2017 Federal Tax Returns
  5. 2016 Federal Tax Returns
  6. 2017 W-2s 
  7. 2016 W-2s
  8. Most recent bank statement
  9. Budget worksheet
At the time of your appointment, please also bring your most recent 1 month of paystubs (last 2 if paid bi-weekly, 4 if paid weekly)
Please email your documents to staff@ahphome.org  We encourage you to use a marker to cross off the first 5 digits of your social security number on any documents, leaving the final four.  When you come to your first appointment, you'll receive a copy of our business disclosure, privacy policy and counseling agreement.  This information is found here:  AHP Disclosures.
